UCI Disclosure Form - Continuing Medical Education: Anyone engaged in content development, planning or presentation must complete this form. Persons who fail to complete this form will not participate in the CME activity.

Handout Permission Release Form: Permission form to place handout material on the SAEM website. 

Audio Video Release Form: The SAEM Annual Meeting may be video taped for future educational and academic use.  By signing this form you are agreeing to release to us for our use, any audio or video tape / still images of any and all materials which you may be represented in. 

Standardized Disclosure Practices: Pursuant to the requirement of a Continuing Medical Education meeting, financial relationships or other relevant conflicts of interest need to be disclosed.  This also pertains to research presentations.  The acknowledgement of conflicts of interests is essential and relevant to the evaluation, interpretation, and application of research discoveries.
