Institution Application
To apply for SAEM approval of your institutional research fellowship program, please view the Institutional Research Fellowship Program Application [pdf].
- Application fee is $400. Please make check payable to "SAEM" and reference "Research Fellowship Application."
- Checks can be mailed to: SAEM, 2340 S. River Rd., Suite 208, Des Plaines, IL 60018 or faxed using the credit card form [pdf] to (847) 813-5450.

Fellow Registration
To register a fellow from an SAEM-approved site, please complete the Fellow Registration Form [pdf]. Fellows must be members of SAEM to register. Registration Deadline: September 16, 2013
Progress Reports
Registered fellows will need to submit an annual Progress Report [doc].
Report Deadline: August 1, 2013
Final Report
Upon completion of an SAEM-approved research fellowship, registered fellows must complete the Final Report [doc] in order to obtain a certificate along with a copy of his/her transcript. Report Deadline: August 1, 2013
