If the answer to any one of the above is No, the application as submitted is unacceptable and will not be forwarded to the subcommittee chair for review. This page should immediately be faxed to the applicant. If the grant application deadline has not passed, the application may be resubmitted after the above deficiencies are addressed. No extension of the deadline will be granted to allow resubmission in this cycle.
